Our team will respond to you via email with approved authorized forms for sending your Origin 12 (SN FT-XX) as an ATF-approved Short Barrel Shotgun to FosTecH.

NOTICE: The short barrel assembly requires a tax stamp before attaching it to your existing Origin 12 shotgun. Attaching it prior to obtaining your tax stamp is illegal.
